Most common is the aperture getting gummed up. This is caused by the grease in the focus/zoom coils getting too hot and flowing in and onto the aperture. The aperture MUST be completely dry with NO lubricant.
Another cause would be for the linkage to get disconnected at some point in the mechanism.
